SUMMARY: The generation and transmission of forces under the foot was investigated in 67 normal subjects and 124 patients with rheumatoid arthritis. The investigations included the measurement of static foot-ground pressure patterns with a Pedobarograph and the assessment of transmission of maximum load during the gait cycle with instrumented force sandals. Information from both sources was compared with the patients' own subjective assessment of pain and walking ability. Static pressures accurately reflect the position of abnormal pressure generation.

SUMMARY: During an 8-year period, 122 inpatients of a psychiatric hospital sustained fractures of the proximal part of the femur. They were treated by operative as well as by conservative methods and their subsequent mobility, morbidity and mortality have been reviewed. The results suggest that these patients are best managed within their familiar environment, that femoral head replacement arthroplasty is appropriate for high fractures and that the prevention of bedsores in conservatively treated patients is of great importance.

SUMMARY: This paper presents a ratio of the pulse wave form transit times in the proximal and distal arterial tree, produced by a simple, non-invasive, repeatable and reproducible technique. It has been shown to be highly significant in separating normal from disease states and the proximal from the distal lesion. It has been shown to be independent of the effects of age.
In the clinical situation the ratio is of value in the initial assessment of the patient (by separating ischaemic from non-ischaemic limb pain); in elucidation of significant occult proximal lesions in the presence of distal disease; in indicating the type of angiography required (femoral arteriography rather than translumbar aortography in suitable subjects); in following the progress of patients in operative and conservative management régimes; and in the long term follow-up of the natural history of obliterative vascular disease.

SUMMARY Expanded polyethylene foam (Plastazote) is used in the treatment of rheumatoid, diabetic, and leprotic foot disorders. This paper describes a diagnostic use for this material. Two photographic techniques combine to give vivid and quantitative representations of foot deformities which are easily applicable to clinical use. Relief photography uses illumination to create an illusion of solidity in a 2-dimensional photograph. Stereophotogrammetry produces contour plots from stereopairs of photographs of the Plastazote footprint. After use the impressions are trimmed and slipped into the patient's shoes in the same way as any other foam insole. |

SUMMARY: One hundred patients wih rheumatoid arthritis were questioned about the acceptability of the surgical shoes that they had been prescribed for their foot problems. The approach was made from the records of a surgical supplies manufacturer. While 90 per cent experienced good alleviation of symptoms, 50 per cent had complaints regarding fit, comfort and styling. The implications of changes in the supply of shoes and the staffing of orthotic concerns are discussed, and suggestions are made for the direction of future research.

SUMMARY: In a randomized double-blind controlled trial the effect of naftidrofuryl (Praxilene) 200 mg t.d.s., taken for 6 months, was compared with placebo in intermittent claudication. Whilst there was a significant subjective improvement regardless of age or treatment, patients over 60 on naftidrofuryl experienced a more rapid symptomatic relief than other patients. At six months this age group showed a significant improvement over the placebo group. There was no significnat objective evidence of improvement.
